Subscapular abscesses: A literature review and evidence-based treatment guidelines

Background An intramuscular abscess of the subscapularis is a rare phenomenon but important pathology for surgeons to be aware because clinical deterioration can rapid and diagnosis difficult. The presentation often mimics other common shoulder pathologies with subacute pain stiffness. Early diagnosis, antibiotics surgical drainage are critical reduce spread joint destruction. Methods A search PubMed Google Scholar databases identified cases subscapular abscess. Data collected about each case included patient demographics, presentation, pathology, treatment outcome. authors report one additional case. Results from 17 were found, 16 in literature described by authors. Sixteen (94.1%) presented reduced range motion worsening over mean 6.7 days prior presentation. Surgical approaches utilised posterior inferomedial approach, deltoid-pectoral approach inferolateral approach. Discussion conclusions From limited data available regarding abscess, make following recommendations: (1) Empirical covering Staphylococcus aureus +/− methicillin-resistant aureus, (2) indicated all cases; (3) tendon-sparing access an most locations within space.
